Resolution

Consider installing the Audit Trail module to have a bigger picture of the backend activity at any time.

More details in the following link from the link:

Sitefinity documentation, Audit trail module: https://www.progress.com/documentation/sitefinity-cms/overview-audit-trail-module

Consider as well, alternatively or as a complement, logging every entry for the problematic form by subscribing to the IFormEntryCreatedEvent event. E.g:

protected void Application_Start(object sender, EventArgs e)
{
Bootstrapper.Bootstrapped += Bootstrapper_Bootstrapped;
}

private void Bootstrapper_Bootstrapped(object sender, EventArgs e)
{
EventHub.Subscribe<IFormEntryCreatedEvent>(evt => IFormEntryCreatedEventHandler(evt));
}

private void IFormEntryCreatedEventHandler(IFormEntryCreatedEvent eventInfo)
{
var entryId = eventInfo.EntryId;
var userId = eventInfo.UserId;
var username = eventInfo.Username;
var ipAddress = eventInfo.IpAddress;
var submissionTime = eventInfo.SubmissionTime;
var formId = eventInfo.FormId;
var formName = eventInfo.FormName;
var formTitle = eventInfo.FormTitle;

var controls = eventInfo.Controls;

if (formName == "sf_form1" ) // or Form Title, or Form ID
{
// feel free to include in the Log any other value captured above.
Log.Write(string.Format("There is a Form response for {0} with EntryID = {1} on {2} ", formName, entryId, submissionTime), TraceEventType.Information);
}
}

Sitefinity documentation, Forms Events: https://www.progress.com/documentation/sitefinity-cms/for-developers-forms-events#iformentrycreatedevent

The above will log any information specified from the submitted entry and form and it will be possible to find the Form entry by its ID in the database in case the issue reappears.

Workaround

Notes

If the site is load balanced make sure the implementation related to the FormEntryCreatedEvent is deployed to every node.
